Policies Agreement & Consent to Performance Consulting
You, or a family member, are about to become involved in a performance consultation with a trained consultant. We wish to take this opportunity to welcome you and also to state some basic principles we believe essential in establishing a good consulting relationship. Please read through this information, asking questions as needed.
FIRST SESSION: During your first session, assessment of limitations, fears, and goals regarding your situation will be discussed.
APPOINTMENTS: Each session is approximately 55 minutes. At the end of each appointment, you can discuss future appointments with your consultant. It is your responsibility to schedule all sessions. If you do not cancel your appointment, without prior notice, you will be charged a missed session fee of $30.
CANCELLATIONS: If you need to cancel a session, please give at least 24hrs notice. We do understand emergencies and will work to help reschedule with notice.
TECHNOLOGY: Online sessions will take place via Zoom. If the client or consultant has technological connection issues, we will reschedule the appointment at no cost.
PAYMENTS: Payment is made when you sign up for our services. We accept credit card and debit card payments. If your payment is declined you will be notified. You will not be able to complete sessions until your payment method is updated and payment is made.
INSURANCE: Our services are self-pay. We do not accept insurance payments. Payments for services received through KLK Performance Consulting are ultimately the client’s responsibility.
THERAPY VS PERFORMANCE CONSULTING: I have a Graduate level degree and am currently in the dissertation portion of my Ed. D in Counseling Education and Supervision. I also have over 20 years of experience as a coach and athlete. While my training is in clinical mental health, and I use techniques from psychology and counseling, I am not a mental health therapist in this relationship. The main focus of these sessions are to analyze performance limitations, such as fear and anxiety, and develop skills and mechanisms to overcome these limitations to be able to achieve peak performance. This framework allows us to do performance consulting without assigning a clinical diagnosis or developing a record of clinical treatment. We will provide clinical/counseling therapy referrals if needed or requested.
RESULTS: All clients will understand their fears and limitations and develop skills to help overcome them.
CONFIDENTIALITY: All information regarding the specific nature of your consultation session is maintained by KLK Performance Consulting and is considered confidential unless specified by a client or parent in writing. However, if there is concern for client well-being, parents and/or emergency contacts may be notified. I reserve the right to use specialty consultation with professionals, adhering to confidentiality standards, as deemed necessary. Although I am not providing counseling services, I follow the American Counseling Association (ACA) Ethics Code.
CONSENT TO CONSULTATION: Your scheduling of a session and your payment indicate that you have read and agree to the above terms and that you give your consent to consultation for yourself or your family member.
